We are currently seeking an experienced RN Case Manager to join our healthcare team. The ideal candidate will have strong clinical skills, be highly organized, and possess excellent communication skills. This is a full-time position with no weekend shifts required.
Location: Kingston, NY
Pay: $42.00 per hour
RN Case Manager Requirements:
- Current RN license in the state of New York
- Bachelor's degree in nursing preferred
- Minimum of 2 years of nursing experience
- Experience in case management preferred
- Strong clinical skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficient in electronic medical records
- BLS certification
RN Case Manager Responsibilities:
- Assessing patients' needs and developing care plans
- Coordinating patient care with other healthcare professionals
- Educating patients and families about their health conditions and treatment options
- Monitoring patients' progress and adjusting care plans as needed
- Maintaining accurate and complete medical records
- Participating in quality improvement activities
